VA Commences Clinical Study Testing Psilocybin for Severe Depression
The Department of Veterans Affairs (VA) has initiated a new multi-site clinical trial to evaluate the safety and therapeutic efficacy of psilocybin for veterans suffering from treatment-resistant major depressive disorder, including those with concurrent post-traumatic stress disorder (PTSD).
Formally titled "Psilocybin Intervention for Veterans Overcoming Treatment-Resistant Depression" (PIVOT), the study is designed to explore novel care pathways for individuals who have not found relief through conventional psychological or pharmacological interventions.
Trial Design and Methodology
As outlined in the ClinicalTrials.gov registry (NCT07226232), the PIVOT trial is a randomized, double-blind controlled study that will compare outcomes across different dosage levels among participating veterans. Participants will undergo structured psychological preparation, guided administration sessions within controlled healthcare environments, and post-session integration therapy led by trained clinical facilitators.
Researchers will evaluate patient-reported depression scores, adverse effect tolerability, and changes in overall mental health status over multi-month follow-up periods.

“Far too many veterans are living with mental health conditions that don't respond to available treatments,” noted VA Secretary Doug Collins, adding that the department is actively pursuing new therapeutic avenues to offer meaningful relief.
As noted in a HMP Global Learning Network report, the PIVOT study joins 20 other active clinical trials supported by the VA that focus on evaluating psychedelic compounds for psychiatric care. While the U.S. Food and Drug Administration (FDA) has designated psilocybin as a "breakthrough therapy" to expedite clinical evaluation, the compound remains investigational. The VA emphasized that clinical implementation outside of formal research settings will occur only if full FDA approval is granted.
